Vaccinations
2) VACCINATIONS
All guests must be current on: Bordetella, Rabies, and Distemper / Parvo. Dogs +6 months must be spayed / neutered. ​

Please email a copy of your pup's vaccinations to records@packmentalitydog.com or bring a copy to your trial.
1) PRE-TRIAL FORMS
The forms below are mandatory to complete before your trial.
3) BOOK YOUR TRIAL
Drop your pup off for a 1 hour trial to allow us to supervise how they interact with our other members. Please note: Daycare is not for all dogs! ​​

If they are happy and comfortable, we will send you an account through which you can book boarding and daycare!
