Vaccinations
Proof of vaccinations are required for all of our guests. Please email a copy of your pup's vaccinations to records@packmentalitydog.com. You may also bring a copy to your trial.
Pre-Trial Forms
All Forms Below Are Mandatory Before Your Trial. Forms Not Needed For Just Dog Walking. Please Take A Few Moments To Fill Out Each Form To Ensure Your Pup's Safety And Happiness. All Forms Must Be Filled Out Before Your Trial.
Vaccines-We require all of our guests to be current on: Bordetella, Rabies, and Distemper/Parvo. We also recommend but do not require the Leptospirosis and Canine Influenza Vaccines. Other: All of our guests over six months must be spayed/neutered.
